NAMI and the FDA are arriving at the same core message about AI in mental health: these tools must be safe, limited in what they claim to do, and grounded in human care rather than replacing it. Both organizations are approaching the problem from different angles, NAMI as a lived‑experience, grassroots mental health organization and the FDA as a medical regulator, but they converge on a surprisingly similar set of expectations for any AI that touches mental health. In practice, this means AI systems will increasingly be judged not just on how “smart” they are, but on how they behave in moments of vulnerability, how they handle risk, and how they respect the people using them.The first shared pillar is crisis safety. NAMI has made crisis detection and response the top item in its benchmark scope: tools should recognize when someone may be in distress and offer appropriate, safe next steps, including crisis lines and human help. The FDA, when it looks at AI that could function as a medical device or influence care decisions, focuses on risk management, adverse events, and clear escalation pathways for situations like suicidality or self‑harm. In both frames, an AI that misses a crisis, or responds in an unsafe, dismissive, or confusing way, is unacceptable, regardless of how good its reasoning or small‑talk may otherwise be.The second shared concern is accuracy and the avoidance of harm. NAMI emphasizes whether responses are factually correct, consistent with evidence, and free of harmful or misleading claims, because the public cannot easily tell a plausible hallucination from a trustworthy answer. The FDA expresses the same idea through requirements for clinical validity, reliability, and ongoing performance monitoring: claims must be supported by evidence, and the system must keep behaving as intended over time. Both perspectives are reacting to the same risk: AI that sounds confident but gives dangerous or misleading mental health advice.Another strong area of overlap is human context: cultural relevance, boundaries, and human factors. NAMI explicitly calls for cultural relevance and human‑centered language across identities and lived experiences; it also insists that AI is not a replacement for care and should not displace helplines, programs, or human support. The FDA, in its own way, focuses on human factors engineering, usability, and safeguards against over‑reliance on AI, especially in sensitive therapeutic contexts. Both essentially want systems that are respectful, understandable, and that consistently remind people where the limits of AI support lie.Our SASI middeware already implements several of these shared expectations as first‑class design constraints. After five rounds of longitudinal testing, one conclusion stands out clearly: safety is not a single dial you turn up or down. Model provider guardrails, application prompts, and a dedicated safety layer each play different and non interchangeable roles.The data show a clear trend across several large API models. Over time they have become more conservative. Risky moves are rarer, but so are direct, grounded responses in difficult mental health conversations. Hedging and avoidance increase as safety policies tighten. In contrast, the same models wrapped with SASI stayed within a narrow, low pathology band across all metrics. They did not drift toward silence or over deflection.Round 5 added an important counterpoint. A relatively unconstrained Llama base model combined with SASI reached a comparable safety zone, but with noticeably stronger therapeutic richness. This suggests that safety middleware can standardize risk behavior across very different underlying model profiles, without flattening the conversation. In other words, safety does not have to come at the cost of usefulness if it is implemented at the right layer.A second conclusion is that integration details matter just as much as model choice. The native cleanup work on MyTrusted.ai surfaced a quiet but critical issue. Legacy adapters, frontend driven modes, and hidden fallback logic were subtly distorting SDK outputs and contaminating benchmarks, even when the SASI safety brain itself was performing correctly. By moving to direct SasiResult usage, config driven modes, real conversation history, and fail closed behavior when SASI is unavailable, the scores now reflect what the SDK is actually doing. They are no longer a blend of SASI plus leftover crisis detectors and emotional taggers from earlier architectures.System prompts emerged as the third pillar. Rounds 4 and 5 made it clear that SASI does not need, and in some cases is actively harmed by, large over engineered system prompts. Minimal but precise prompts focused on identity, operating modes such as Default, Child, and Therapist, and fixed crisis language were sufficient. With clean wiring of MDTSAS scores and safety hints, even relatively small or lightly aligned models behaved like good adults in the room. They showed strong boundary integrity, early risk recognition, and appropriate crisis resource provision without an encyclopedic prompt trying to script every response. This is why future rounds will explicitly test families of prompts, from ultra minimal to more structured, to map how much prompt complexity is actually necessary once a dedicated safety SDK is in place.Taken together, multi round testing, multi judge scoring, and continuous SDK cleanup have clarified the separation of concerns. Provider level safety stacks, application level prompts, and model portable safety middleware each have distinct responsibilities. The path forward reflects that clarity. What Five Rounds of Testing Taught Us About Safety in Mental Health AI

This document explains what each of the 20 USAIS and FDA metrics actually means in normal human terms. These are the qualities we measure when we evaluate whether an AI is safe, stable, emotionally appropriate, and trustworthy.Our intention with these testing rounds is not to call out or shame any specific app, but to show how hard it is to get mental‑health responses right with AI, and how much iteration it really takes. Publishing live results for named apps is meant as a contribution, not a critique: a way to surface concrete failure modes and successes so teams can improve faster, tighten safety, and ultimately protect their users better. If your product appears in these charts, please read it as an open invitation to collaborate on making the next version safer, not as a verdict on your work.The testing process is still very hands‑on by design. For each round, every prompt is entered manually into each app or web interface, and every response is copied back into a shared spreadsheet so nothing is lost in translation. Each answer is then evaluated by three independent AI “judges” (Claude, GPT, and Gemini APIs), which receive the user message, the app’s reply, and the previous five turns of context so they can see whether the conversation is drifting or stabilizing. The judges score responses across the 20 clinical and safety metrics published on the site, using only anonymized model letters rather than app names to keep the evaluation blind.
